has the ability to ensure continued excellence and innovation in behavioral
health services within Summit County and the surrounding region.
Donations made to Community Support Services, Inc., a 501(c)(3)
not-for-profit organization can enrich programs such as case
management, residential, homeless outreach, facility improvement
or any other area in which a donor specifies.
Individuals or corporations wishing to donate to Community

A great way to recognize someone special
in your life is to give a gift in their honor or memory. Birthdays,
anniversaries and graduations are wonderful occasions to honor someone with a
tribute gift. There are also those times when you want to thank someone or
simply give a gift because you care about them. By making a gift to Communtiy
Support Services, Inc., you can honor a special occasion, pay tribute to someone
you care about or celebrate the memory of a loved one - all while helping adults
with severe and persistent mental illnesses on their journey through Recovery.
Community Support Services Inc. will send you an acknowledgement of your tax
deductible gift and will inform anyone you designate of the tribute gift,
without specifying the amount of the gift. For example, if you make a tribute gift
in honor of or in memory of someone, you may wish to have family members notified.
Honor and memorial gifts are also recognized in our newsletter,
annual report and on our web site.
When you make a gift, just let us know:

Who the gift is in honor of or in memory of.

Why you are making the gift, if appropriate (i.e., their birthday, wedding, etc.).

The address and name of the person who should receive the acknowledgement (we do not indicate amount of the gift, only that a gift was made).
You may use this form to provide us with the information about your gift.
Community Support Services is a 501(c)(3) not for
profit organization and all gifts are fully tax-deductible
within the limits of the law.
