GETTING STARTED

If you or someone you love would like to receive mental health services, the first step is to begin the intake process by calling our appointment call center at 330-996-9130 or fill out the form below. If possible, the person in need of services should be available for the initial call.

What to Expect at Your Intake Appointment

Your appointment with the Intake Specialist will take place in a private office. The Intake Specialist will ask you questions about why you have come in for services and any symptoms you have experienced (mood swings, difficulty sleeping, feeling sad, etc.) You will be asked about your goals for treatment, as these are different for everyone. We will work together to determine what these goals are for you.

What to Expect After Your Intake Appointment

When your intake appointment is complete, you will be scheduled for appointments at Community Support Services. If the Intake Specialist determines that another agency's services are a better fit for your specific needs, we will work with you to schedule an appointment.

What to Bring to Your In-Person Appointment

  • Your driver's license or state ID card
  • Your personal information - address, phone number and Social Security number
  • Your Medicaid, Medicare or other insurance card
  • A list of all medications you are currently taking
  • If you have received mental health services in the past, name and contact information of previous provider(s)

What to Have Available for Your TeleHealth Appointment

  • Your personal information - address, phone number and Social Security number
  • A list of all medications you are currently taking
  • If you have received mental health services in the past, name and contact information of previous provider(s)

NOTE: If you are missing any of the things listed above, CSS will help you obtain what is needed. Do not let this stop you from scheduling and attending your appointment!
